All athletic forms (besides the physical) can now be filled out and returned digitally as well as printed and turned in to the Main Office. If you decide to submit forms digitally, please click the link, fill out the form, save to your computer with your changes with your child's name as the document name, and email the PDF to Katlyn Putney at putneyk@tmore.org.
